AFFIDAVIT OF DISCREPANCY

I, JOHNNY RUNAYA RONQUILLO, Filipino, of legal age, married, residing at

Leones East, Tubao, La Union, after being sworn to in accordance with law, depose and
say that:

1. I am known by the name “JOHNNY RUNAYA RONQUILLO” and have used the
same in my personal and official records and interactions, as shown by the following
Voters’s Identification Card (a copy of which is hereto attached as Annex “A”):

2. In my Certificate of Marriage under Registry No. 2003-62 celebrated on April 8,

2003, my name was recorded as “JHONNY RUNAYA RONQUILLO,” which is not
the correct spelling of my name (a copy of which is hereto attached as Annex “B”);

3. I am the father of JOHN LLOYD VALDEZ RONQUILLO, born on January, 29,

2004; and JOVA VALDEZ RONQUILLO, born on June 27, 2012;

4. In the Certificate of Live Birth of John Lloyd, my name was recorded as “JHONNY
RUNAYA RONQUILLO,” which is not the correct spelling of my name (a copy of
which is hereto attached as Annex “C”);

5. In the Certificate of Live Birth of Jova, my name was recorded as “JHONNY

RONAYA RONQUILLO,” which is not the correct spelling of my name (a copy of
which is hereto attached as Annex “D”);

6. The foregoing entries may have been the result of a typographical error on the part of
the person who prepared the same;

7. That the entries, “JOHNNY RUNAYA RONQUILLO” and “JHONNY RONAYA

RONQUILLO” refer to one and the same person, the Affiant; and,

8. I execute this affidavit to attest to the foregoing facts.
