I, Daniel T. Gestopa Jr.Father of Danielyn Gestopa a fourth-year student taking up Bachelor of

Elementary Education in the College of Education of Negros Oriental State University Bais City
Campus II, do hereby allow my daughter to undergo the off-campus practice teaching as part of
requirements to complete and finish the degree.

As to my knowledge,she would have her practice teaching this second semester of school year
2022-2023 at Bais City Pilot School and that I understand that the school and the cooperating
school authorities shall not be liable for whatever accidents, nor responsible for any untoward
incident that may befall on her in the course of student teaching due to her negligence and
failure to follow policies, rules,regulations, imposed on her by Negros Oriental State University
and Bais City Pilot School.
