Incident Report on Employee’s Unsafe Practice

Summary

On September 10, 2022, the Electrical Engineer in-charge of the safety audit of the two major
kilns in the R&R Cement-Rizal Plant was reported for unsafe practice for committing violations
against the Health and Safety Manual of the plant. During the audit, the Electrical Engineer
allegedly failed to comply with the proper lock-out/tag-out of one of the two kilns which resulted
in a minor incident in the plant. Without the immediate reporting by the assisting personnel
during the audit, another employee could have started the maintenance procedures with the kiln
still operating. The Plant Health and Safety Team called upon the erring Engineer for
appropriate action and the incident was communicated across the plant employees to serve as
reminder to always follow the Standard Operating Procedures (SOP) for lock-out/tag-out.

Description

The SOP for machinery imposes strict compliance with the Health and Safety Manual of the
Plant considering the risks that the personnel are exposed to during its operations. The
lock-out/tag-out measures, specifically for the kilns, are significant steps in making sure that
maintenance will be undertaken while all operations are stopped to prevent the danger during
the maintenance check of the machines. The SOP, however, was not followed by the Electrical
Engineer in-charge when it intentionally missed the locking-out/tagging-out of one of the kilns
when he assumed that it was not turned on in the first place. He cleared the checklist without
physically examining the machine. This is in clear violation of the Company Rules and could
have caused a major safety risk in the plant.

Outcomes

The action of the Electrical Engineer in-charge resulted in a minor incident. This was mitigated
by the assisting personnel who were diligent in re-checking the machines prior commencing the
maintenance procedures.

Conclusion

The Plant Health and Safety Team called upon the erring Engineer for immediate apprehension.
His clear violations shall be meted out the appropriate disciplinary action to prevent these kinds
of incidents from occurring. For information, the incident was communicated across the plant
employees to serve as a reminder to always follow the SOP for lock-out/tag-out. The Team also
set mandatory refresher training on the Plant SOP as part of its health and safety initiatives.
