SEPARATE ELECTRIC METER

I, ANALIZA GALENDEZ- ALBA, Filipino, of legal age, married and a

resident of District 2, Brgy. Canito-an, Cagayan de Oro City, after having
been sworn to in accordance with law, hereby depose and say, that:

1. I am the lawful and registered owner of a parcel of land

situated at Prk.4, Brgy. Mabuhay, Baungon, Bukidnon. A copy
of proof of ownership is hereby attached.

2. I authorized, MARIA REGINA GALENDEZ, Filipino, of legal

age and a resident of Prk.4, Brgy. Mabuhay, Baungon,
Bukidnon to apply for a separate electric meter under
Bukidnon Second Electric Cooperative, Inc.(BUSECO);

3. This authorization is executed upon the request of the

applicant for purposes of applicant’s application of a separate
electric meter located at Prk.4, Brgy. Mabuhay, Baungon,
Bukidnon.
