AFFIDAVIT-COMPLAINT
We, NICASIO D. RAAGAS and FELICITACION A. RAAGAS, 81 and 84
years of age, respectively, both Filipinos, and residents Brgy. San Pedro
Pasakay, Tunga, Leyte, under oath, hereby state:

1. That we are formally filing a complaint for Violation of Expanded

Senior Citizens Act of 2010 and related abuses in the law against
Department of Social Welfare and Development (DSWD) in the
Municipality of Tunga, Leyte;

2. That having no source of income, the DSWD included us in the list

of beneficiaries of the Social Pension Program for Indigent Senior
Citizens in 1960;

3. That after receiving pension for three (3) months, we were unjustly
removed from the list of Senior Citizen Beneficiaries;

4. That on December 2021, we went to the DSWD Office in the

Municipality of Tunga, Leyte to inquire our status as beneficiaries
and we were told that the same will be rectified immediately;

5. That until today, no action has been conducted by the DSWD in

Tunga, Leyte to rectify or include us in the list of beneficiaries;

6. That we are executing this Affidavit-Complaint to attest to the

truth of the foregoing and for whatever other legal purpose this
may serve.
