Letter of Consent

I hereby authorize Fujitsu Consulting India Private Limited (FCIPL) to communicate with all or
any of my former employers, Educational Institutions or any other professional references for the
purpose of verifying information and/or obtaining an account of my work experience, skills and
education.

I hereby declare that the information provided by me is correct as per the best of my knowledge. I
acknowledge and understand that if any of the documents / details provided by me at the time of
offer lead to a discrepant background check (Report color RED), FCIPL holds the right to withdraw
/ cancel the offer made to me with immediate effect.

This Authorization may be signed in counterparts, each of which shall be deemed an original, but
both of which taken together shall constitute one and the same instrument.
