SELF-DECLARATION FOR GETTING DGCA ENDORSEMENT POST COMPLETION A320 TYPE RATING FROM
GULF AVIATION ACADEMY

I, Mr./ Ms. _____________________, resident of India, hereby affirm and declare that after the
successful completion of A320 Type Rating at Gulf Aviation Academy (GAA), I will take the responsibility
to coordinate with DGCA of India for all license endorsement activities subject to GAA completing all the
endorsement activities (Documentation, etc.) as per the set DGCA rules & regulations for ATOs.

If there is any delay by DGCA India in endorsing the license, I take personal responsibility to manage the
same, subject to GAA completing all the necessary documentation for DGCA endorsement and provide
necessary information if there are any queries/shortfalls which is directly related to GAA.

As I am taking responsibility for DGCA license endorsement request GAA to provide all the necessary
help & support to successfully complete the endorsement.
