1. INTRODUCTION

1.1. PROJECT SCOPE OF SP_18 AND SP_21 TANKS REFURBISHMENT

Maintenance and Repair of SP_18 and SP_21 tanks as per recommendation of SGS Inspection
report and shared BOQ including replacement/repair of shell plates, roof plates, bottom
plates, weld build-ups, structure, Spiral stair case and roof handrails, pipes and fittings, nozzles
& appurtenances, scaffolding supply, fabrication, erection, installation, testing, grit blasting,
painting / coating, calibration and commissioning of tanks.

2. SCOPE AND BASIS OF CONSTRUCTABILITY STUDY

The scopes of Constructability Study are based on the available drawings, Scope of work,
site survey, project experience in oil and gas sector, and several references of similar
projects. Constructability is a set of method prepared to explore possibilities of
construction, and highlights of difficulties including potential risk and hazards to be
anticipated during the Tanks construction and repair. It includes detailed construction
planning and engineering work to support the repair and construction activities for Tanks
SP_18 & SP_21.

3. METHODE OF CONSTRUCTABILITY STUDY


The work steps of conducting this constructability study are:
• Identify all construction activities which will be obstruct with existing facilities and
simultaneous with operation.
• Collect data of existing drawings and does the necessity of site survey, to identify the obstacles
will be occurred during construction activities with the existing facilities.
• Prepare basic method, and investigate any difficult activity.
• Highlight any difficult activities and simulate the activities by 2D/3D model
• Confirm or change unfeasible method and re-simulate.
• Estimate the duration of each activities and completion of project
• Prepare interim report.
• Prepare final report.

4. CONSTRUCTION STUDY OF
Constructability program is the application of a disciplined, systematic optimization of the
construction related aspects of a project during the planning, design, procurement, construction,
tests and start-up phases by knowledgeable, experienced construction personnel who are part
of a project team. The program’s purpose is to enhance the projects overall objectives. These

objectives establish the framework for the entire project and must be kept in mind by all
projects team members when evaluating each of the various constructability factors.
An experienced construction personnel need to be involved with the project from the earliest
stages to ensure that the construction focus and experience can properly influence the owners,
planners and designers, as well as material suppliers. The construction person should be a full-
fledged member of the project team, with access to, and participation in, the early decisions
that affect to the project. This influence includes those considerations that would reduce the
overall project schedule, improve overall project quality, operability, maintainability and
reliability, and would reduce the overall life-cycle cost and most of all focusing on safe
executions strategy.
There are several factors to be considered such as:
• Project Schedule
• Material Procurement
• Safe Working Execution
• Site Survey
• Commissioning of Tanks
• Inspection of Tank (Hydro Testing, DPT, PAUT)
• Hand over of tank

5. PERMITTING
Work permit will be crucial and specify in project milestones. Therefore, the knowledge of the
types of work permits is prevailing in facilities. The knowledgeable of work permit application is
very important to be known by the PTW holder. There are a window time that must be
considered during the specific work activity. It should be noted the work permit when the
working is interfere with existing facilities. In the meaning the time available is utilized properly
and work progress achieved in timely manner.
Before starting work on Tanks following steps will be followed to seek permission for start of
work
1. Entry Pass Application for staff / workers
2. Complete training/ course before entering site including FTW
3. List of equipment to be brought to the site. (Appendix-1)

7. TRANSPORTATION ROUTE
Transportation route is divided into 2 locations; inside the Shell terminal and outside the
terminal working area between Pre Fabrication Workshop to terminal. Inside terminal
Route is agreed with operations and other stake holders. Most of the vehicle will be used
for transportation of Shell Plates, Roof Plates, Pipe Spools, Pre fabricated parts, civil
material and other construction material to inside terminal and outside terminal. To
ensure the safe vehicle movement all the drivers will be trained on defensive driving by
well reputed 3rd party trainer and the entire driver will follow dedicated Route show in
below figures 1. Before entering to terminal all the drivers will take gate pass from
guardhouse.
7.1. INISIDE TERMINAL
Transportation route inside the terminal is shown in figure 1. Red highlighted line is
dedicated route for vehicle movement and blue line is dedicated route for pedestrian
movement as well as dedicated for vehicle use in case of emergency. Pedestrian route
will be segregated by scaffolding and sign-ages. This route strategy is already aligned with
operations during constructability review. Parking area for vehicles is already mapped on
layout and highlighted in yellow colour.

7.2. INSIDE TERMINAL FOR TANK SP_18

To access SP_18 tank following route will be used for all vehicle movements. Portion of Dike
Wall for SP_18 will be demolished to access tank yard and it will reinstatement after completion
of work. Crane working area also marked in red colour. Red highlighted line is dedicated route
for vehicle movement and blue line is dedicated route for pedestrian movement as well as
dedicated for vehicle use in case of emergency. Pedestrian route will be segregated by
scaffolding and sign-ages. This route strategy is already aligned with operations during
constructability review.

7.3. INSDIE TERMINAL FOR TANK SP_21

To access SP_18 tank following route will be used for all vehicle movements. Crane working area
also marked in red colour. Red highlighted line is dedicated route for vehicle movement and
blue line is dedicated route for pedestrian movement as well as dedicated for vehicle use in case
of emergency. Pedestrian route will be segregated by scaffolding and sign-ages. This route
strategy is already aligned with operations during constructability review.

7.4. OUT SIDE THE TERMINAL

Outside terminal route is dedicated for vehicles movement from Pre Fabrication Workshop
KMW Port Qasim to Shell Terminal. On this route, vehicle shall shift the material Workshop to
Shell terminal. Blue line highlighted the route. All drivers will follow the NHA speed limit and
having DDC training.

8. SPECIAL CONSTRUCTION EQUIPMENT


The construction activities inside terminal, for lifting and shifting of Roof plates, Shell
Plates. Curb Angle, Wind Girder & other Structure material etc will be performed by
Mobile crane, Boom Truck & Fork Lifter. All Equipment will be 3rd party certified and
drivers having valid licence and DDC training.

11. OBJECTIVES OF THE REVIEW


 The review of Constructability is essential to ensure that all aspects of the work are
considered such that there all methods, sequence, equipment, logistics (vehicles, routes),
SIMOPS, interfaces, layout(storage/warehouse), welfare, risks and controls are identified
and understood, support from stakeholders (Terminal Ops and Parco Terminal) during
the construction period are confirmed. This will provide the Construction Contractor
with a firm basis to proceed and should minimise any delays in mobilisation.
 The investigation of the optimum sequencing of work packs will ensure that the
construction is carried out ahead of key construction activities (such as HDD, PARCO
internal works) in the most logical manner. This will ensure that there are no stoppages
due to conflicts in work activities.
 Opportunities to reduce the expected duration of individual work packs or of the whole
construction period will be identified during the review

13. CONSTRUCTION ACTIVITY

13.1. LOGISTICS:

All material/equipment will be mobilized at site in 20/40 feet flat bed trailer and Hi AB
with appropriate arrangements of tie-off with ratchets, D rings and wire rope. Only
dedicated and trained driver will be assigned for the job having DDT and valid driving
license. Speed limit will be followed not exceeding to NHA (National Highway Authority).
For vehicle entry within the terminal, DEL must get permission from Terminal prior to
entry and inspection of the vehicle will be done at Terminal gate. Inspection check list
will be shared with PTW application and after PTW issuance, vehicle will be guided by a
competent and trained Flagman. The route of the vehicle has been assessed for any
road/ ground restrictions. Proposed route is adequate for vehicle turnings, but reversing
is not allowed. Route is map up on layout and attached as appendix 2 to 6.
For offloading of trailer, Crane of 25/50 tones capacity (As per site requirement)/Fork
Lifter will also follow the same entry protocols as mentioned above and crane will move
with with closed boom. Material segregation will be done accordingly. Site walkabout
made involving all stakeholders for safe manoeuvring of vehicle considering possible
blind spots on the proposed route, underground utilities, above ground utilities, narrow
turnings, escape route in case of emergency, and any reversing scenario. No such issues
involved.
Collision, rupture/damage, crush and LOF can be the worst credible consequences while
L&H is captured in next step. All controls are captured in HAZID for same project.

13.2. LOGISTICS FOR OUTSIDE WORKING:

All material/equipments for Pre Fabrication works will be mobilized at KMW Port Qasim
workshop in 20/40 feet flat bed trailer and Hi AB with appropriate arrangements of tie-
off with ratchets, D rings and wire rope. Only dedicated and trained driver will be
assigned for the job having DDT and valid driving license. Speed limit will be followed not
exceeding to NHA (National Highway Authority). Inside KMW workshop Vehicle will be
guided by a competent and trained Flagman. The route of the vehicle has been assessed
for any road/ ground restrictions. Route is map up on layout and attached as appendix 2
to 6.
Material offloading will be carried out by Hi Ab crane/Fork Lifter as per site requirement.
Material segregation will be done accordingly. Site walkabout made involving all
stakeholders for safe manoeuvring of vehicle considering possible blind spots on the
proposed route, underground utilities, above ground utilities, narrow turnings, escape
route in case of emergency, and any reversing scenario. No such issues involved.Collision,
rupture/damage, crush and LOF can be the worst credible consequences.

13.3. SITE PREPARATION FOR SP_18, SP_21 AND STORAGE AREA INSIDE THE
TERMINAL:

For SP_21 site preparation hard barricade will be provided to restrict the access
towards nearby tank SP_20. Temporary Storage area inside SP_18 tank yard will be hard
barricade. As per prior approval from SPL, designated Store container, storage area &
Diesel Storage area will be hard barricade. Safety Dashboard will be installed in this area
having PPEs, ERP, HSE Policy, HSSE KPIs and safety bulletin shared in posters. Emergency
exits will be marked, and escape route will also be highlighted for better understanding
of staff. All relevant posters will be available within this area. Vehicle route will be
marked. For welfare facilities, all arrangement is there and mapped up on the layout
having rest area, dining area, washroom facilities and drinking water.

13.4. MOBILIZATION OF CORE TEAM

Core Team will be mobilized after HSE Induction and required training . Plan date for
mobilization should be from 25th Sep to 10th Oct.

13.5. HSE INDUCTION AND TRAININGS

SPL and DEL will conduct HSE Orientation and other training as per TNA.

13.6. SHIFTING OF STORE AND OFFICE CONTAINER

Office Container( 40 ft) and Store Container (20 ft) will be mobilized at site in 40 feet flat
bed trailer with appropriate arrangements of tie-off with ratchets, D rings and wire rope.
Only dedicated and trained driver will be assigned for the job having DDT and valid
driving license. Speed limit will be followed not exceeding to NHA (National Highway
Authority). For vehicle entry within the terminal, DEL must get permission from Terminal
prior to entry and inspection of the vehicle will be done at Terminal gate. Inspection
check list will be shared with PTW application and after PTW issuance, vehicle will be
guided by a competent and trained Flagman. The route of the vehicle has been assessed
for any road/ ground restrictions. Proposed route is adequate for vehicle turnings, but
reversing is not allowed.
For offloading of Containers, 50 Ton Crane will be used. It will also follow the same entry
protocols as mentioned above and will move with with closed boom. Site walkabout
made involving all stakeholders for safe manoeuvring of vehicle considering possible
blind spots on the proposed route, underground utilities, above ground utilities, narrow
turnings, escape route in case of emergency, and any reversing scenario. No such issues
involved. Collision, rupture/damage, crush and LOF can be the worst credible
consequences. All controls are captured in HAZID for same project.

13.7. PROCUMENT OF MATERIAL

Bill Of Material will be calculated as per shared Tender drawings BOQ, it will be shared
with SPL for acknowledgement. All material MTC, Data Sheets & Vendors detail will be
shared with SPL. Pre Inspection will be carried out by Q/C personal before delivery at
site. For imported material all documents will be shared with SPL for approval. All
materials will be shifted to KMW workshop and Shell terminal through proposed route
same as mentioned above.

13.8. SHIFTING OF SCAFFOLDING MATERIAL, TOOLS AND EQUIPMENTS.

Scaffolding material, Tools and equipments ( Generators, Compressors, Mixer Machine &
Winch Machines etc) will be mobilized at site in 20/40 feet flat bed trailer with
appropriate arrangements of tie-off with ratchets, D rings and wire rope. Only dedicated
and trained driver will be assigned for the job having DDT and valid driving license. Speed
limit will be followed not exceeding to NHA (National Highway Authority). For vehicle
entry within the terminal, DEL must get permission from Terminal prior to entry and
inspection of the vehicle will be done at Terminal gate. Inspection check list will be
shared with PTW application and after PTW issuance, vehicle will be guided by a

13.9. SET UP OF STORE AND STORAGE YARD

As per prior approval from SPL, Store container and temporary storage area & Diesel
Storage area will be set up near SP_18. Also temporary storage areas will be set up inside
tank yards for and area will be hard barricade and all control measures should be
considered as shared in Risk Assessment document and HAZID session.

13.10. THIRD PARTY CERTIFICATIONS OF TOOLS AND EQUIPMENTS

All tools and equipments will be 3rd party certified. They must be used by a trained and
competent person. Right tool for the right job. It must be inspected on regular basis. All tools &

Equipments must be regularly maintained and serviced to ensure that all are in good working
condition. Ensure tools bags are used and a 3-point contact is maintained.

13.11. WELDING /HOT WORK:

For all welding/ Hot work activities 3rd party certified, and pre-inspected equipment shall
be in placed (welding machines, grinders, electrical distribution boxes, baking oven,
electrode holder, Genset, welding leads, welding holders, PPE’s). Before starting hot
work job area will be cleaned for all combustible material, dedicated fire watch shall be
assigned on spot with at least 2 fire extinguishers for each hot work activity. Canopy to
be provided to contain sparks generated by welding. LEL monitoring shall be there.
Atmosphere tests ( O2, LEL, Gas Test), Rescue Plan & presence of Stand By Man and all
other protocols need to be followed during hot work inside Confined Space.

Fire, explosion and fatality are worst credible consequences for the activity and detailed
list of mitigation is available in HAZID for same project.

For working in tank Roof & Curb Angle, Shell, Wind Girder, Spiral Stair case, Bottom
Annular Plates & its allied structures gas testing to be performed prior to entry and no
permission will be granted until working environment within tank becomes safe for
working and meeting requirements in term of Oxygen level, LEL and other gases as per
their TLV. If tank is not ready for confined space due to unhealthy environment,
ventilation to be provided by putting EX-rated blower on manhole.
As PTW made available for activity, gas cutting to be done from Gas Cutting set with
Torch from outside of the tank having 2 flashback arrestors on torch and 2 on DA
(dissolved acetylene) cylinder. Both Oxygen and acetylene cylinders will be 3 rd party
certified and pre-inspected. Whole area where hot work activity planned will be covered
by Fire Blanket. All hot work protocols will be applied same as above mentioned.
Confined space entry will be done for welding pass from inside the tank and installation
of pre-fabricated plates, angles and pipes.
For working within confined space, only trained staff will be appointed fully aligned with
way of communication, emergency exits, role & responsibilities in case of emergency. A
competent and trained Hole Watcher will be appointed for communication and
supervision. A dedicated fire watch will be assigned with 4 fire extinguishers at site and in
easy access. In case of emergency the ERP will be followed.
Fire & explosion are worst credible consequences for the activity and detailed list of mitigation is
available in HAZID for same project.

13.12. PRE FABRICATION WORKS


Pre fabrication workshop will be set up at KMW Port Qasim. After pre inspection of material by
SPL at vendors facility, material will be shifted to Workshop as per proposed route plan. 20/40
Ft flat bed trailer/Boom Truck shall be used for transportation and offloaded with Boom Truck.
Plates for Tank shell, Tank roof, Wind Girder Angle and Curb angle shall be inspected and
Cutting, Grinding, Bending and Rolling activities will be carried out. Rolled plates and angles

shifted to Grit Blasting yard for blast and primer application. All pre fabrication works will be
inspected and approved by SPL.All protocols for Hot Work/ Welding will be followed as
mentioned above.

13.13. ERECTION AND DISMANTLING OF INTERNAL AND EXTERNAL SCAFFOLDING

After issuance of Permit to work all scaffolding material will be shifted inside terminal through
proposed route both for SP_18 & SP_21. Initially for SP_18 scaffolding material will be offloaded
in designated temporary storage area and later on will be shifted inside Tank Yard. For SP_21
material will be shifted inside Dike Wall with the help of Crane as per approved Rigging study.
Trained and competent staff will be assigned for work. All Tools, Equipments and Scaffolder
should be 3rd Party Certified. Pre Inspection of material will be carried out on regular basis.
Scaffolding should be installed as per approved Design and will be inspected by SPL. Tagging
system will be adopted and inspected by SGS certified Inspector. Detail of hazards/Risks and
control measures will be discussed in HAZID session and shared Risk Assessment.

13.14. DISMANTLING OF EXISTING SP_18 DIKE WALL

Existing dike wall for SP_18 will be demolished partially to make access inside tank yard for
shifting of material and movement of crane inside yard. Approximately 3.5 m wall be demolished
and surface preparation will be performed. After issuance of work permit Dike wall will be
demolished with Hilty Machine, Hand tool and grinder. All protocols for hot work will be
followed as mentioned in serial no , Debris will be removed from site with wheel borrow to
designated area and later on will be disposes off outside the terminal. For this area for wheel
barrow movement shall be free from any obstruction in order to avoid Slip / trip hazard. Wall
demolishing shall be done as per drawing / specification and witness by the contractor Civil
Supervisor and SPL representative.

13.15. CONSTRUCTION OF POND SEPARATION WALL


After issuance of civil work permit area for separation wall will be marked and surface
preparation will be carried out. Material will be shifted with wheel borrow . Manually/ hand mix
concrete shall be shifted and poured with the help of wheel barrows and hand tools. For this
area for wheel barrow movement shall be free from any obstruction in order to avoid Slip / trip
hazard. Mason will be deputed for levelling / finishing of concrete. Surveyor / civil supervisor
shall continuously check the elevation of lean for proper alignment of pipe supports. Skilled
labours and mason shall be deputed for safe working.
13.16. DISMANTLING AND ERECTION OF TANK ROOF AND ALIED STRUCTURE

External scaffolding erection will be inspected by SGS inspector to access Tank roof and its allied
structure. After issuance of Permit to work all area will be cordon off and pre inspection will be
carried out for SP_18 & SP_21 tank roof and its allied structure. Marking and identification of
roof plates will be done as per SPL confirmation. Hot Work/welding protocols will be strictly

followed same mentioned as above. Reinforcement for Curb Angle must be done & it will cut in
6m lengths. Cutting of roof plates, Curb Angle, Man holes, Circulation vents and reinforcing pads
will be done by Gas Cutter and Grinders ( 4”, 7& 9’’). All tools and equipments should be 3 rd
party certified and pre inspection will be done.
Pre fabricated plates, angles, man holes & circulation vents shifted inside terminal from KMW
workshop as per approved transportation route with Flat Bed Trailer/Boom Truck, offloading will
be carried out by Boom Truck and Fork Lifter. Logistics protocols mentioned in serial no 13.1.
For SP_18 Crane will be parked inside tank yard to reach different roof locations. For SP_21
crane will be parked outside the tank yard and all crane working will be accessed from this
location. It will be used to lift the material on top of roof as per approved L & H plan and rigging
study. Material tie off with Slings, D shackles, web belts and offloaded at top of tank roof.
Parallel activities will be performed for roof, curb angle and circulation vents and weekly plan
will be submitted to SPL for approval. For marked plates & angles cutting activity will be
performed. Tacking of plates and welding of plates & curb angle will be carried out as per
drawings and entire satisfaction of SPL. Inspection of plates must be done for all tacking and
welding activities. All protocols of hot work will be in place as mentioned in step 13.11.

13.17. E & I WORKS


After issuance of work permit, LOTO will be applied and Locks & Tags will be in placed by
both Shell & DEL. Isolation plan at every point of isolation will be shared with SPL.
Isolation of Power and instruments signals will be performed by Instrument team.
External Instruments (Servo Gauge Type Level transmitter, Temperature Transmitter
along with probe cables and Local Temperature Gauge etc. will be removed by DEL team
under the supervision of SHELL instrument team and will be handed over to SHELL team.
After recalibration of instruments by SPL, installation will be carried out by DEL
instrument team as per entire satisfaction of SPL.

13.18. DISMANTLING AND ERECTION OF WIND GIRDER


External scaffolding erection will be inspected by SGS inspector to access Wind Girder and its
allied structure. After issuance of Permit to work all area will be cordoned off and pre inspection
will be carried out for SP_18 Wind Girder and its allied structure. Hot Work/welding protocols
will be strictly followed same mentioned as above. Cutting of wind girder angle, Backing plates
and Gusset Plate will be done by Gas Cutter and Grinders ( 4”, 7” & 9’’) and other hand tool as
per site requirement. All tools and equipments should be 3 rd party certified and pre inspection
should be done by Site Supervisor. Parallel activities like Shell Plates cutting, Roof plates cutting
and Spiral Stair case cutting planned along with dismantling of Wind Girder. Daily meeting
between DEL and SPL should be conducted to cross check parallel activities hazards and their
control measures. Dismantled material will be shifted to designated area in tank yard. Surface
preparation will be done prior to installation and should be inspected by SPL.
Wind Girder angle cutting, rolling and welding will be done in Pre Fabrication work shop and
inspected by SPL. It will be shifted inside terminal from KMW workshop as per approved
transportation route with Flat Bed Trailer/Boom Truck, offloading will be carried out by Boom
Truck and Fork Lifter. Logistics protocols mentioned in serial no 13.1

Wind Girder, Backing Strip and Gusset plates installation will be carried out as per drawing and
entire satisfaction of SPL. All protocols of hot work will be in place as mentioned in step 13.11

13.19. DISMANTLING AND ERECTION OF SPIRAL STAIR CASE STRUCTURE

External scaffolding erection will be inspected by SGS inspector to access Spiral Stair Case, Plate
form and its allied structure. After issuance of Permit to work all area will be cordoned off and
pre inspection will be carried out for Spiral Stair Case and its allied structure. Hot Work/welding
protocols will be strictly followed same mentioned as above. Cutting of Spiral Stair Case, Stringer
Channels, Platforms, Grating and its allied structure will be done by Gas Cutter and Grinders ( 4”,
7” & 9’’) and other hand tool as per site requirement. All tools and equipments should be 3 rd
party certified and pre inspection should be done by Site Supervisor. Parallel activities like Shell
Plates cutting, Roof plates cutting and Wind Girder, Annular Plates, Spiral Stair case and other
structure cutting planned to fall under Simultaneous Operation and all protocols for SIMOP
strictly followed and discussed in daily TBT. Daily meeting between DEL and SPL should be
conducted to cross check parallel activities hazards and their control measures. Dismantled
material will be shifted to designated area in tank yard. Surface preparation will be done prior to
installation and should be inspected by SPL.
Spiral Stair Case cutting, rolling and welding will be done in Pre Fabrication work shop and
inspected by SPL. It will be shifted inside terminal from KMW workshop as per approved
transportation route with Flat Bed Trailer/Boom Truck, offloading will be carried out by Boom
Truck and Fork Lifter. Logistics protocols mentioned in serial no 13.1.
Spiral Stair Case, Support Channel, Columns & Grating installation will be carried out as per
drawing and entire satisfaction of SPL. All protocols of hot work will be in place as mentioned in
step 13.11.

13.20. DISMANTLING AND ERECTION OF SHELL PLATES & ANNULAR PLATES.

For working in tank, gas testing to be performed prior to entry and no permission will be
granted until working environment within tank becomes safe for working and meeting
requirements in term of Oxygen level, LEL and other gases as per their TLV. If tank is not
ready for confined space due to unhealthy environment, ventilation to be provided by
putting EX-rated blower on manhole.
As PTW made available for activity, gas cutting to be done from Gas Cutting set with
Torch from outside of the tank having 2 flashback arrestors on torch and 2 on DA
(dissolved acetylene) cylinder. Both Oxygen and acetylene cylinders will be 3 rd party
certified and pre-inspected. Whole area where hot work activity planned will be covered
by Fire Blanket. All hot work protocols will be applied same as above mentioned.
Confined space entry will be done for welding pass from inside the tank and installation
of pre-fabricated diffuser along with supports of diffuser. Diffuser will be transported to
the relevant area on trailer and off loaded by crane within dyke wall of respective tank

and will be mobilized through tripod, chain block and wheel barrow. For inside tank
External Scaffolding, tripod and chain blocks will be used.
For working within confined space, only trained staff will be appointed fully aligned with
way of communication, emergency exits, role & responsibilities in case of emergency. A
competent and trained Hole Watcher will be appointed for communication and
supervision. A dedicated fire watch will be assigned with 4 fire extinguishers at site and in
easy access. In case of emergency the ERP will be followed.
Fire & explosion are worst credible consequences for the activity and detailed list of
mitigation is available in HAZID for same project.

13.21. DISMANTLING AND ERECTION OF PIPING AND NOZZLES

External & Internal scaffolding erection will be inspected by SGS inspector to access Internal
Piping, Nozzles and Reinforcement pads form and its allied structure. Cooling Water Line, Tank
circulation vents, Still Well, LT, TT & LSHH, Gauge Hatch, Inlet Diffuser, Tank Foam Purer and
External Piping will be dismantled & and cutting carried out with Gas Cutter and Grinders ( 4”, 7”
& 9’’) and other hand tool as per site requirement. All protocols of hot work will be in place as
mentioned in step 13.11.
Internal Piping, Nozzles, Cooling Water Line, Tank circulation vents, Still Well, LT, TT &
LSHH, Gauge Hatch, Inlet Diffuser, Tank Foam Purer and External Piping installation will
be carried out under supervision of Person In Charge as per drawings and entire
satisfaction of SPL. All protocols for welding/hot work strictly will be followed during
installation of piping & Nozzles. After completion of all construction activities, pipeline
shall clean by removing all the welding particles and debris using pig with the help of
compressor.
Pipe Spool, Nozzles, Supports & Reinforcement pads pre fabrication works will be carried out at
KMW workshop and after inspection by SPL will be shifted inside Terminal. It will be transported
from Pre Fabrication workshop to site. For this purpose, 20/40 feet long flat bed trailers/Boom
Truck and 50-ton capacity crane/Fork Lifter shall be utilized or as per requirement or site
condition. To avoid damage to pipe bevel end and to external polythene coating, suitable slings /
belts with rubber lines lifting hooks shall be used. Rubber pads shall also be placed on trailer bed
and in between the two layers of pipes to avoid damage to external coating.
The pipes shall be stacked on sand bags to avoid direct contact with ground. Pipes
stacking shall be done considering all L&H protocols and approved L&H plan shall be
followed. Only 3rd party certified and pre-inspected crane with 3 rd party certified and
trained operator and 3rd party certified, pre-inspected lifting gears will be engaged. 3 rd
party certified, and trained rigger will be assigning for the offloading activity. Area must
be cordoned off continuous monitoring shall be there. A competent and trained person
will lead the activity as Person In-Charge who is responsible to follow approved L&H plan.
Falling object, Suspended load, crane collapse, collision and LOF are the major hazards
relating to the activity. All controls are captured in HAZID for same project.

13.22. TIE-INN JOINTS:

After installation of pipe line Tie-Inn joints shall be performed inside SPL. Mechanical
Supervisor shall inform to SPL Operation and Engineering team regarding hot work in
operational area / inside terminal. Field Weld Joint shall be performed at riser inside
Terminal. Proper canopy cover with GI sheets / Fire blankets will made to execute this
job. LEL will be continuously monitored by authorized gas tester, check and record by
HSSE Supervisor before start of job. Permit shall be obtained from SPL operation before
commencement of activity.
Field joint surface preparation shall be done by means of power brushing at site and
painting shall be done at site

13.23. SURFACE PREPARARTION BY GRIT BLASTING

Surface Preparation for Tanks Internal & External , its allied structure and piping will be done by
Grit Blasting. Internal & External surface areas will be accessed by scaffolding. All piping spools
and pipes grit blasting will be done at Pre Fabrication Workshop and inspected by SPL.
All machine, tools,hoses and equipment should be 3 rd party certified. Always wear the proper
safety gear whenever you are in the grit blasting work area, even if you are just passing through.
This includes wearing gloves, safety goggles, boots and coveralls. Make sure that your safety
gear has been approved for use with sandblasting, as not all types are able to withstand the
pressure. Area should be barricade, ensure proper PPEs as per activity, Availability of
manufacturing certificates, Authorised persons to operate grit blasting machine.

safety gear whenever you are in the painting work area, even if you are just passing through.
This includes wearing gloves, safety goggles, boots and coveralls. Make sure Area should be

painters will perform the job. Risk of eye injury, Falling from height, Slip/trip and falls hazards
will be discussed in daily TBT.

monitoring shall be there. Atmosphere tests ( O2, LEL, Gas Test),
Rescue Plan & presence of Stand By Man and all other protocols need to be followed
during painting inside Confined Space. Fire, explosion and fatality are worst credible
consequences for the activity and detailed list of mitigation is available in HAZID for same
project.

13.24. EXTERNAL AND INTERNAL PAINTING OF TANKS

After surface preparation and hydro testing of Tanks painting will be carried out as per approved
painting brands and entire satisfaction of SPL. Area will be accessed by External scaffolding. Paint
will be stored as per MSDS. For better understanding MSDS will be discussed with staff. All
machines, tools,hoses and equipment should be 3 rd party certified. Always wear the proper

safety gear whenever you are in the painting work area, even if you are just passing through.
This includes wearing gloves, safety goggles, boots and coveralls. Make sure Area should be

13.25. CIVIL WORKS FOR SP_18 & SP_21

a) MARKING AND MANUAL EXCAVATION FOR PIPE LINE SUPPORTS, CROSS OVER,
DIKE WALL AND POND SEPARATION WALL:
Initially Existing services will be checked by DEL Team. Construction team would
conduct an investigation and acquire information about site services might have
been placed/embedded below the ground. Services, wherever found shall be
rerouted and protected to place will not affect site activities. Surveyor would
conduct the site survey to mark/stake-out the lines and levels of the objective
excavation for foundations as indicated in the approved drawing.

Manual excavation shall be commenced for pipeline supports, cross over, Dike Wall
& pond separation wall. Area shall be cordoned off having competent supervision.
Slopping shall be provided to avoid cave in. Digging shall be done up to 1.2 meters
deep. Considering the depth of digging, confined space protocols shall be
implemented. Gas testing shall be performed before commencement of job.
Competent & trained Stand by Man shall be appointed. Maximum coverage of body
shall be advised considering the bio hazards. Snake catchers and snake bite kit shall
be available with certified & trained 1 st aider. Safe means of access and egress
(Ladder) shall be available for digging work Spoil/loose soil will be stacked outside
but at least 1 meter away from excavation. Serious injury leading to fatality is worst
credible consequence for the activity and controls are in place in HAZID for same
project.

b) DISMANTLING OF EXISTING STRUCTURES


Dismantling of existing structures including trimming of pipe support foundations, QFT tank
foundation area, platform, crossover, pits, cables, platforms, etc. complete in all respect. It also
includes disposal of waste material as per SPL requirement. DEL shall provide certificate from
local authority of Solid Waste Management Company for safe disposal.

c) EXCAVATION

Excavation for foundations in all kinds of soil, dry or wet, hard or soft, including
backfilling of excavated earth in plinth i/c breaking clods, dressing, watering and
consolidating by ramming in layers not exceeding 150mm depth to obtain 95%
modified AASHTO density including lifting, shifting or as per instruction and
specification of the consultant and approval of client (Site Engineer). DEL will carry
out compaction test as per specification with out for Fast Flush Tank, Pipe supports,
Pipe crossovers, Operating platform, Existing pond separation wall. Manual
excavation shall be commenced for pipeline supports, cross over, Dike Wall & pond
separation wall. Area shall be cordoned off having competent supervision. Slopping
shall be provided to avoid cave in. Digging shall be done up to 1.2 meters deep.
Considering the depth of digging, confined space protocols shall be implemented.
Gas testing shall be performed before commencement of job. Competent & trained

d) STONE SOLLING
After completion of excavation work manually laying 150mm thick stone soling of approved
quality under foundations, floor including hand packing vertically and filling voids with spawels,
consolidating and compacting with mechanical means as per drawings. Approved size stones
shall be shifted inside excavated area with help of wheel barrows. For this area for wheel barrow
movement shall be free from any obstruction in order to avoid Slip / trip hazard. Stone soling
shall be done as per drawing / specification and witness by the contractor Civil Supervisor and
SPL representative.

e) PLAIN CEMENT CONCRETE

After completion of stone soling activity concrete mixing shall be done as per
drawing /specification for Fast Flush Tank, Pipe supports, Pipe crossovers, Operating
platform, Existing pond separation. Approved size of PCC 1:4:8 (1 cement 4 sand and
8 coarse aggregate). Manually/ hand mix concrete shall be shifted and poured with
the help of wheel barrows and hand tools. For this area for wheel barrow
movement shall be free from any obstruction in order to avoid Slip / trip hazard.
Mason will be deputed for levelling / finishing of concrete. Surveyor / civil
supervisor shall continuously check the elevation of lean for proper alignment of
pipe supports. Skilled labours and mason shall be deputed for safe working.

f) REINFORCED CEMENT CONCRETE

After completion of PCC, Reinforced cement concrete shall be developed 28 days


cylindrical strength of 3000psi, using approved quality 3/4" max. size graded
crushed aggregate & fine aggregate including approved form work. It should be

done as per drawing /specification for Fast Flush Tank, Pipe supports, Pipe
crossovers, Operating platform, Existing pond separation. Mixer machine/Manually
mix concrete shall be shifted and poured with the help of wheel barrows and hand
tools. 6in dia and 12" long cylinder tests will be carried out by from approved
laboratory. For this area, wheel barrow movement shall be free from any
obstruction in order to avoid Slip / trip hazard. Mason will be deputed for levelling /
finishing of concrete. Surveyor / civil supervisor shall continuously check the
elevation of lean for proper alignment of pipe supports. Skilled labours and mason
shall be deputed for safe working.

g) STEEL REINFORCEMENT

Laying of hard grade ribbed deformed (min yield strength 60,000 psi) steel
reinforcement bars by competent steel fixer for Fast Flush Tank, Pipe supports, Pipe
crossovers, Operating platform and Existing pond seperation wall. For this area
movement shall be free from any obstruction in order to avoid Slip / trip hazard.
Trained and skilled Mason will be deputed for levelling / finishing of. Surveyor / civil
supervisor shall continuously check the elevation of lean for proper alignment.
Skilled labours and mason shall be deputed for safe working.

h) BITUMEN COATING

For Fast Flush Tank, Pipe supports, Pipe crossovers, Operating platform and Existing
pond separation wall Bitumem coating 1200 micron thick will be carried out as per
drawing and specification and as directed by the SPL. For this area movement shall
be free from any obstruction in order to avoid Slip / trip hazard. Trained and skilled
Mason will be deputed for levelling / finishing.

i) PIPE SLEEVES, NON-SHRINKAGE GROUT AND STEEL EMBEDDED PARTS.

For Fast Flush Tank, Pipe supports, Pipe crossovers and Operating platform fixing of
MS or sch. std. pipe with seam and leak proof sealant complete in all respect and to
the entire satisfaction of Engineer incharge as per drawings. Also laying of 25 mm
thk. non-shrinkage grout, having minimum compressive strength of 40 N/mm2 of
FOSPAK and SIKA or equivalent as approved by client in accordance with the
Company specification, as per drawings and instructions of SPL. For Steel Embedded
Channels, M.S plates, Anchor lugs, Angles, Bolts with double nuts and washer etc.
Will be fixed in concrete.
For this area movement shall be free from any obstruction in order to avoid Slip / trip hazard.
Trained and skilled labours and Mason will be deputed for work.

j) JOINT FILLER AND WRAPPINGBAND

Joint Filler "STOPAQ 4200 FILLER" having density 1.2-1.5 g/cm³ (NEN 1833) and
Wrapping Bond "STOPAQ WRAPPINGBAND EZ" 300mm wide in combination with
other STOPAQ materials should be applied on Tanks foundation as per drawings and
entire satisfaction of Supervisor. For this area movement shall be free from any
obstruction in order to avoid Slip / trip hazard. Trained and skilled labours and
Mason will be deputed for work.

k) TANKS FOUNDATION REAPIR AND APPLICATION OF JOINT SEALANT & WRAPPING


STOPPER

Tank foundations will be repaired by applying plaster with 1:3 cement sand mortor
(1 cement 3 sand) ratio, including making champhered edges, rounding of corners at
junctions recesses & racking- out-joints & curing complete etc. after chipping off and
application of joint filler and wrapping band for tank foundation repair as per
specification & instruction of the Consultant and approval of Client. For this area
movement shall be free from any obstruction in order to avoid Slip / trip hazard.
Trained and skilled labours and Mason will be deputed for work.

l) BLOCK MASONRY AND PLASTERING

Good quality blocks having standard size and smooth plaster with 1:3 cement sand
mortor (1 cement 3 sand) ratio will be applied as per instruction and entire
satisfaction of Civil In charge and approval of client. Mixer and block will be shifted
by wheel barrow. For this area, wheel barrow movement shall be free from any
obstruction in order to avoid Slip / trip hazard. Mason will be deputed for levelling /
finishing . Surveyor / civil supervisor shall continuously check the elevation of lean
for proper alignment. Skilled labours and mason shall be deputed for safe working.

m) MODIFICATION OF EXISTING STRUCTURES

Modification of existing structures including crossover platforms, walkways etc. will be complete
in all respect as per SPL requirements. It includes re-installment and re-instatement of any
structure as per original condition after dike wall height extension as per drawing, instruction
and specification of the Consultant and approval of Client.
For this area movement shall be free from any obstruction in order to avoid Slip / trip hazard.
Trained and skilled labours and Mason will be deputed for work.

n) BACKFILLING

Backfilling shall commence using similar excavated material. Care shall be taken
to remove bolder, vegetation and any other deleterious materials. The thickness

of the back fill shall not be greater than 300mm. Back filling followed with
compaction shall continue till final level is achieved.

13.26. NON-DESTRUCTIVE TESTING:

Each welding joint shall be performed by 3rd party certified Welders as per drawings and
will be inspected by QA/QC inspector and SPL. All pre fabricated material will be
inspected QA/QC inspector & SPL prior to installation. Visual inspection of all operations
for Fit up and welding of Roof plates, Shell plates, Annular plates, Wind Girder Angle,
Spiral Stair case, Internal & External piping and allied structure starting. Physical
condition of welding machine, welding tools / tackles and specification of the Each and
every welding joint shall be inspected.
DPT, VBT & PUAT of the weld joints for plates, structure and piping as per the relevant
code requirement shall be conducted after welding operation by approved 3rd party NDT
Company. For this purpose, the area shall be cordon off up to the specified safe limits.
The repair of weld joints if observed shall be rectified as per approved welding repair
procedure and the repair segment of the weld shall be done inspected and tested by
approved 3rd party Inspector.

Personal Injury, Fatality and Falling from height are the worst credible consequences and
controls are captured in HAZID of same project.

13.27. HYDRO STATIC TESTING OF TANKS:

After completion of all construction activities, both tanks SP_18 & SP_21 shall be hydro
tested. The system being tested shall be totally filled with approved water and properly
vented to exclude air pockets. Water will be hold for 12 Hrs at 25%, 50%,75% and 100%
level to test the tank.
Water will be filled with 50000 litres capacity bowsers. The filling point shall be at the
top of tank through Foam Pourer line or any other line as agreeD by SPL. Temporary pig
launcher & receiver installed at both point for Filling & de-watering purpose. Preferably a
high-volume centrifugal pump shall be used to ensure continuous filling of water into the
system. The total length of the line is 250 meters approx. and total water filled in the
system is approx. 13230000 Litters for SP_18 & 1,583,362 Liters for SP_21. Adequate
settling time (minimum 4 hours) should be provided to allow the equalization of
temperature of fill water and that of the ground. A positive pressure shall be induced in
the system by continuous pumping and the air shall be allowed to vent out from high
vents to purge entrapped air.
After the completion of test, de-pressurizing shall be done gradually and under
controlled conditions, water shall be disposed at the end.

After successful hydro test of tanks, it will be flushed with the help of Pig removing all
water from the tank. Decanting shall be done through bowsers and will be dispose off
outside the terminal.
After complete dewatering all systems should be thoroughly drained and blown with air
using compressor for at least 2 hours to minimize residual moisture after completion of
hydrostatic testing.
Only 3rd party certified compressor shall be utilizing, and pre-inspection shall be done
prior to start work. Dedicated staff shall be assigning for compressor and hard barricades
shall be provided. All joints shall be secured, and calibrated gauges shall be installed for
vigilant supervision.

13.28. HYDROTESTING FOR PIPE LINE

Initially the line will be pressurizing up to 100 psi and hold for settling time. The pressure
shall be applied gradually or increased in steps; typically, in steps of 100 psi until the
required test pressure is reached (as per client specification). Calibrated Pressure /
temperature recorder is to be used and a log sheet shall be maintained for manual
recording of pressure and temperature gauge reading at every one-hour interval.
Buried lines shall be preferably tested for 24 hours and above ground for 4 hours.

13.29. DEMOBILIZATION OF TOOL, EQUIPMENT AND MANPOWER:

For demobilization, it is required to provide facility as maintained as handed over before


the project execution. Housekeeping shall be considered all the way and all temporary
facilities must be dislodged and removed from terminal. DEL must submit their vehicle
movement plan with the terminal authorities for permission of their vehicles for
demobilization tools and equipment.
