AFFIDAVIT OF UNDERTAKING

I, JERYLOU AGAS Filipino, of legal age, married and a resident of Bacolod City, after
having been sworn to in accordance with law hereby depose and say:

That I am the bonafide Atty-in-fact of MR. ELIZE BEAT, the registered owner of a
commercial building located at Magsaysay St, Bacolod City;

That I am the one processing for the Occupancy Permit of the said commercial building;

That the Bureau of Fire Protection required the owner to install an Automatic Fire AIarm
on the said commercial building;

That I hereby undertake to complete the installation of the necessary Automatic Fire
Alarm on the said commercial building upon processing of the Business Permit for the calendar
year 2022;

That I am executing this affidavit to attest the veracity and truthfulness of the
foregoing.
