To begin with, the first management failure was despite clear signs stating that the crane must be operated only by trained workers but still it was operated by three people, who are not trained in the usage of a crane. In addition to this, the general workshop manager did not believe that the workers must be responsible for health and safety and even they do not have time for it. This approach of the general working manager is incorrect because it is the responsibility of every employee working in the workshop to recognize any discrepancy in safety measures and they must report it immediately if something is not found correct. Furthermore, it must also be realized that the workers regularly informed the frequent near misses which involved the overhead crane and as a result of this immediate action should have been taken to resolve the issue as early as possible but no action was taken.
