PARENTAL/GUARDIAN CONSENT AND WAIVER

TO WHOM IT MAY CONCERN:

The undersigned, being the parent/guardian of

_______________________________________, who is enrolled at Valencia National High
School of Grade ________ Section ___________, hereby state that;
1. I am giving my consent for my son/daughter to attend to the conduct of Kaamulan
Festival Drum and Lyre Competition at Malaybalay City on February 28-March 01,
2018.

2. I am aware that the mentioned activity is part of the school’s extra-curricular activity and
I hereby consent thereto;

3. I am aware of that my child will be ferried by a bus chartered by LGU-Valencia, as their

mode of transportation to and from the above-mentioned area, and that he/she will be
billeted at Bukidnon State University-Secondary School Laboratory and I hereby consent
thereto;

4. I am aware that the following activity will be made/conducted by them with the
assistance of __________________________ as their class adviser/subject teacher/band
coordinator.

And I hereby consent thereto;

5. Having been appraised and properly informed of the foregoing, I hereby grant the right to
their teacher to do, with proper diligence, any legal action necessary to preserve the
wellbeing, safety and security of my child and his/her companion during the conduct of
the said activity.

6. Having been appraised and properly informed of the foregoing, I hereby declare that I
will not hold the teacher, school management, the Division Office and the Department of
Education for any incident that may occur beyond their control.
