Application for ‘Basic Police Clearance’

If you are of British nationality, or have been residing in the UK (England, Scotland, Wales and Northern Ireland)
in the last 5 years, then you will need to provide Seabourn Cruise Line with a police clearance check, which you
have to do online following the below instructions. Once you have the police clearance, please send a copy to
your recruiter/coordinator so that we can update our record. Please keep the receipt of the clearance, as you
will be able to claim the cost of this back when you join a ship. You do not need to complete the form below.

All others nationalities, please complete the form below on page 3, and return only. Please do not apply for
anything online.

Seabourn Cruise Line Limited, as applicable (“the Company”), will obtain a criminal record report on you in
connection with your employment by, or if not working as an employee, your engagement (including
volunteer or independent contractor assignments) with the Company. These reports may be obtained at
any time after receipt of your authorization and, if you are hired or engaged by the Company, throughout
your employment or engagement period. TalentWise Solutions LLC (“TalentWise”), a consumer reporting
agency, will obtain the report for the Company. Further information regarding TalentWise, including its
privacy policy, may be found online at www.TalentWise.com. TalentWise is located at 19800 North Creek
Parkway, Suite 200, Bothell, WA 98011, USA, and can be reached at (866) 338-6739 (U.S. toll free number).

The information that may be included in your report includes: Social security number trace (as applicable),
criminal records checks, public court records checks, driving records checks, drug tests, educational records
checks, and licensing and certification checks. The information contained in the report may be obtained from
private and/or public record sources, including sources identified by you in your job application or through
interviews or correspondence with your past or present co-workers, neighbours, friends, associates, current
or former employers, educational institutions or other acquaintances. You may, upon written request made
within a reasonable time after receipt of this notice, request disclosure of the nature and scope of any such
report.

AUTHORISATION

Company and its designated representatives for the purpose of assisting the Company
