Dear Sir/Madam;

My name is Kading, and as a part of the Contact Tracing Team, I am requesting a temporary
allowance for the volunteer work we have done in a span of 15 days, starting from the 1 st of August until
the 15th. We have continued our work regardless of our contract being terminated.

The team worked hard to complete their assigned tasks, and deliver good service on time. The
tasks assigned to me in the specified date is only to report the Team’s accomplishment to the DILG
Office. I was present at the RHU office from time to time, and at the DILG Office as well.

I hope I can humbly request an allowance to the Municipal Office regarding of our volunteer
