DATE AND TIME OF EMERGENCY: October,28 ,2021, Thursday at 08:30 AM

LOCATIONS: JCPDI I-2004 C Line
EMERGENCY SCENARIO – Describe the possible Emergency Scenario
One of the crew will report to Entry attendant that one employee is injured inside Confined Space at C
Line. The dummy will be used for rescue. Body Harness will be fixed with dummy and pulled out by Tripod
device or Boom Truck, on the top of the roof. After that the one crew will act as Injured Person (IP) on the
roof. ERT team will shift the IP to clinic for further treatment.
CAUSES – How could this emergency occur
Suspected Injury in the confined space, due to trip hazard and Injured person could go up by ladder.
WORST CASE CONSEQUENSES – Describe reasonable worst-case consequences
Oxygen Deficiency, insufficient illumination, vapor accumulation Slip and trip, access block and traffic flow.
EMERGENCY RESPONSE – How will we respond
Entry Attendant will immediately call the nurse for medical assistance, and site safety officer. The site
safety officer will call the Drill commander for the incident and location. Drill commander will inform the
Emergency Rescue team and HSE manager. Then HSE manager will inform to site RC HS&E safety
about the incident. Site nurse will reach the site with ambulance for first aid and other medical assistance.
Site safety will conduct the gas test. Site nurse will take necessary information from entry attendant before
enter the confined space to help the IP. Site nurse will give the necessary medical help to the Injured
Person, (IP) inside confined space. ERT team will rescue the IP (dummy) from the confined space with the
recommendation of site nurse. The IP will shift to site clinic for further medical advice.

Tripod, Dummy, Body harness, Ambulance and Boom Truck if required

PERSONNEL REQUIRED – What personnel roles are required, Identify personnel

Drill commander: Will coordinate with safety manager and ERT team & ensure proper evacuation of IP
with ERT team. He will also perform head counting. He will give clear signal to go back to the job.
Traffic Warden (TC) will check monitor the traffic and ensure that road is clear, and other access clear
from all tripping hazards.
Nurse: Will provide First Aid and will recommend the IP to shift in the clinic
ERT TRAINING – What Training of personnel is required to prepare them to respond to this emergency
Confined Space & Emergency Response Procedure.

ER DRILLS – What drills (type & frequency) could be used to prepare for this scenario
Evacuation from Confined Space.

EQUIPMENT INSPECTIONS – What equipment inspections are required to ensure equipment is ready
and available for immediate use in an emergency (Type & frequency of
