We invited you to participate in a research that is about the behavior of the children at school. This
study involves doing development tests for children to determine their behavior when they are in
school. You will be invited to visit the school premises and to see the behavior of the child.
Questionnaires will be given after the observation.

The enclosed questionnaire has been design to collect data for this study. Your participation is much
appreciated. There is no known risk in your participation.
