This is to permit my son/daughter ___________________________________ to participate/join the

Synchronized School-Based BSP/GSP Scouting Activity to be held on October 22, 2022, at Balabag Elementary
School, Balabag, Malay, Aklan.

I have considered the benefit that shall be derived from the participation of my son/daughter in the Scouting
Activity with the understanding that the necessary safety measures/precaution shall be taken.

However, I shall not hold the BSP and GSP, Aklan Council responsible for any untoward incidents that may
happen beyond their control.
