After Post-Qualification Evaluation made by the Technical Working Group (TWG) and
pursuant to Rule X of the Revised Implementing Rules and Regulations of R.A. No. 9184, the
REGIONAL BIDS AND AWARS COMMITTEE (RBAC) hereby APPROVES the 15 December
2021 Bid Evaluation Report of the TWG which was received by the RBAC Secretariat on even
date, a copy of which is hereto attached and made an integral part hereof. The said TWG
Evaluation Report is hereby summarized as follows:

IN VIEW THEREOF, this RBAC RESOLVED as it hereby RESOLVES as follows:

1. To DECLARE ADVANCE COMPUTER FORMS, INCORPORATED as the bidder with the

SINGLE CALCULATED AND RESPONSIVE BID for the project, Printing and Delivery of
Voters' Information Sheets (VIS) for the May 9, 2022 National and Local Elections for
Region IV-B under Project ID No. 01-2021IVB-VIS:

2. To RECOMMEND to the HEAD OF THE PROCURING ENTITY the issuance of the Notice
of Award to ADVANCE COMPUTER FORMS, INCORPORATED for the project, Printing
and Delivery of Voters' Information Sheets (VIS) for the May 9, 2022 National and Local Elections
for Region IV-B under Project ID No. 01-2021IVB-VIS;

3. To REQUEST the HEAD OF THE PROCURING ENTITY to authorize Regional Election

Director of Region IV-B ATTY. JOSE NICK A. MENDROS to sign the Notice of Award
(NOA), Contract and Notice to Proceed (NTP) on behalf of the Commission on Elections; and

4. To REQUIRE ADVANCE COMPUTER FORMS, INCORPORATED to post its

performance security in an amount equal to a percentage of the total contract price in
accordance with the following schedule:

named bidder which has been declared to have tendered the Single Calculated and Responsive
Bid, and all concerned offices including the Commission En Banc for its information.
