AFFIDAVIT OF LOSS

I, BERNADETTE B. MANTUHAC, Filipino, of legal age, widow and a resident of

San Vicente, Liloan, Cebu, Philippines, after being duly sworn to in accordance with law,
hereby depose and state:

1. That I am a holder of a Metro Rewards card;

2. That I always place the said Metro Rewards card in my wallet for
safekeeping;
3. That just recently, I could no longer locate my wallet where I placed my
Metro Rewards card for safekeeping;
4. That despite diligent search and efforts to locate the said Metro Rewards
card, I could not find the same such that I believe that it is now lost beyond recovery;
5. That I am executing this Affidavit of Loss to attest to the truth of the
foregoing and to support my request for the issuance of new Metro Rewards card in lieu
of the lost one.

IN WITNESS WHEREOF, I have hereunto set my hand this 23th May 2019 at
Cebu City, Philippines.
