a. Section 51 (2) RA 7077 s-91.
b. Circular 1, GHQ AFP, dtd 01 Feb 00, Subject: Active Duty Training of
AFP Reservist.

2. Request that I be allowed to undergo Voluntary Active Duty Training

(VADT) effective upon approval for the following reasons.

a. That I was commissioned into the Reserve Force PAF (AFP), effective
Day__Month__Year, pursuant to circular #4 dated 05 July 2010.

b. That I seek to upgrade my proficiency as regard to the rudiments of the

military in my current assignment as____________________, 1st Airforce Wing
Reserve.

c. That I waived my rights to received pay and allowances during the

duration of my Voluntary Active Duty Training.

3. Further request favorable consideration and approval.
