CASE SUMMARY
Mr.AR, 21 years old single male, single educated up to 9th std, currently working as a driver
from a Lower socioeconomic status from a rural background of Bangalore.
Presenting complaints:
 fearfulness, tension in social situations and gatherings, palpitations, excessive sweating
 low mood, feeling bad about himself
 avoidance of social situations
He reported complaints of fearfulness, tension in social situations and gatherings, palpitations,
and excessive sweating for 2 years. Consequently, he reported feeling low and bad about himself
(I am not going to enjoy like normal people, I will always be under confident, My friends will
eventually forget me as I always make excuses). He also reported avoidance of social situations
and low mood for 6 months. He reported that he used to feel extremely tense and fearful while
going to work since his manager used to speak loudly. He would also worry and felt anxious
about his work, whether he would be able to work without mistake, what if there’s a meeting and
he is expected to talk/ present them. He also reported that he always has a fear of meeting new
people and fear of performing in public. He mostly avoids situations that involve group activities
like meetings or official gatherings which reflect on him badly. If the situation is unavoidable
and he has to attend social situations, he used to feel restless, have excessive sweating and
increased heartbeat, and felt that other people would be able to notice him being nervous which
would further increase his symptoms. There is no history was suggestive of organicity, substance
use, sudden episodes of breathlessness, and palpitations. Personal and Family history: He is the
third born of a non-consanguineous marriage. He was a full-term baby born out of normal
delivery with no delay in developmental milestones Premorbidly: He was slow to warm up as a
child and was shy and introverted. He used to have limited interaction with people except for his
family friends and family. He studied up to 9th std and later discontinued his studies due to not
being interested in studies. His parents used to compare him with his siblings and blame him for
discontinuing his studies. No family history was suggestive of psychiatric illness and physical
illness. Initially, he was doing clerical work in a company, but later quit his job because of his
fearfulness at the workplace especially talking to superiors. Subsequently, he quit his job and he
learned car driving as it required minimal interaction with others and is currently working as a
driver.
