Dear CEO:
have sent several payment instruments to which you have held and not returned back to me
and have failed to zero out my account. | have now filed it with the US Treasury to pay off my
balance using my cestui due vie trust and you are now obligated to file a 1099-OID form to
receive the funds from the U.S. Treasury and failure to do so will result in a corporate violation
of my rights per HJR 192 of 1933 and 15 USC 1692 A-K. Failure to accept my payment as
such is both an illegal and unlawful act by your corporation and is a lienable offense in the
amount of $6,000,000 in which the U.S. Treasury will be contracted with me to collect on my
behalf. It is imperative that this account be paid off within 3 days of receipt of this notice and
all negative reporting to the credit bureaus be corrected as | have certified mail showing proof
that | submitted the payoff balance beginning (enter Date).
Since there was no return of my negotiable instruments you have now accepted 5 payments,
toward this account. Your company has failed to pay off the balance owed, failed to
acknowledge reasons as to why the instruments were not accepted for payment even though
they were not returned which by definition is accepting payment per each letter sent, and have
continued with defamation of character in credit bureau reporting, all while harming a living
soul in the name of First-Middle: Last Beneficiary

also holds possible imprisonment and fines per 18 USC 1701, 18 USC 1702, 18 USC 1703,
and 18 USC 1708. Anyone who tampers with this mail other than the CEO or acting
CEO of this corporation will also personally be named in the lien with the U.S. Treasury.
