Dear sir:

Good day!

I Would like to inform in your good office that the Battery of our service vehicle ISUZU SPORTIVO is
now on weak condition. Primarily it causes hard start and affects the lightings.The said battery was
purchased last JUNE 2019 which showed that the useful life was already more than 3 years.

In this connection, I Would like to request for the procurement of a new battery as a reserve/
replacement of the old one.

I Would like to inform you further that this is included in my PPMP and with existing approved
budget.

For your information and appropriate action.
