SUBJECT : FEEDBACK REPORT RE: TECHNICAL ASSISTANCE FOR CYCLE 12 FOR THE PREPARATION OF SUP-
PLEMENTARY FEEDING PROGRAM, IMPLEMENTATION CYCLE 12 YEAR 2022, PANTABANGAN, NUEVA

Hereunder is a report on the above subject:


I. Objectives
To conduct monitoring and provide technical assistance to LGU on the implementation of Supplemental Feeding Program.

II. Observations/ Findings

The undersigned arrived in the Municipality of Pantabangan, Nueva Ecija at exactly 10:45 am. SF FOCAL person and Day Care Worker were
present. The undersigned introduced himself as Project Development Officer I of the DPEO Nueva Ecija and task to provide a technical assistant
for the preparation of supplementary feeding program implementation cycle 12.
Based on the interview conducted with Mrs. Merlita B. Aquinillo (SF Focal) and Mrs. Joana F. Mamerto (Day Care Worker/ Brgy. Villarica) the fol-
lowing informations were gathered:

1. The Municipality of Pantabangan, Nueva Ecija is one of the Transfer of Fund last Cycle 11 and the condition of their cycle 11 last year was, not able to submit

their liquidation on time.

2. When I visited again the Day Care Center of Barangay Villarica, I was glad they really followed my advice on what they should do and the things to be prepared.

I congratulated Mrs. Joanna Mamerto as the Day Care Worker on the job well done. Yet, the only one I notice was some of Day Care Workers are in confuse in

the manual computation of age in months.

3. The undersigned told ma’am Aquinillo that they should conduct an orientation before the start of cycle 12 to all the Day Care Workers, because on what I have

seen, that there were Day Care Workers who got confused in manual computation of age in months and other activities.

4. The technical assistance we discussed for this coming cycle 12 with ma’am Merlita Aquinillo ( SF Focal) are the following:

 Master list

The master list mas have a content of :

- Province, Municipality, Barangay, Name of Day Care Center/ Child Development Center

- Name of Child

Last Name, First Name, Middle Name

- Sex / Gender

Male / Female

- Height

- Age in Months

- If member of Pantawid Pamilyang Pilipino Program

- Ethnicity

Aetas ,Dumagats, Igorots, etc.

- Dissability / PWD

Child of Solo Parent

Name of Parents / Guardian.

 Masterlist of beneficiaries with nutritional status for height and weght.

 Identification of volunteers.

 Identification of stock room that safe and secured.

 Vehicles needs to transport for goods from the province to municipal to barangay.

4. Regarding the Nutritional Status Summary template I have given Ma’am Aquinillo an example so that she has an idea.

5. The undersigned explained Ma’am Aquinillo about weighing scale which I prefer most the salter and detecto to be used rather than bathroom weighing scale

because of its accuracy and so, I even suggested her that the Day Care Workers can ask for a fund from their Captain to finance the procurement of new weighing

scale to be used.

6. PDO I suggested that before the feeding of the children, they should have their deworming first and give them vitamins. They must coordinate to their Municipal

Health Center on this matter.

7. Also,from the PDO, I informed Ma’am Aquinillo that this coming cycle 12, always be ready especially, in the transportation to be used for the distribution of goods.

8. The Undersigned suggested that their PES should be monitor quarterly their Day Care Parents and Day Care Workers, so that day care parents can learn more about

our program and other matters.

9. The Undersigned instructed Ma’am Aquinillo (SF Focal) to submit Requirements for the cycle 12 that is on or before July 15,2022 and I remind her about the

requirements indicated below.

Requirements for Regional Procurements

 Two (2) copies of Notarized MOU with complete signature per page including the acknowledge page.

 Summarized Master list with Nutritional Status.

Requirements for TOF (Nutri Bun / Milk Bun)

 Four (4) copies of Notarized MOU.

 Summarized Master list with Nutritional Status

 Two (2 ) copies of SB Resolution

Note:

 Separate Copy for Regional Procurement and TOF

 All documents should original copies

 The Requirements to passed the transmittal named to Mrs. Ma. Elena Carreon so that all documents are straight to the SFP unit.

 Deadline of submission in on or before July 15, 2022.

ISSUES AND CONCERNS RECOMMENDATION/AGREEMENTS

Other activities and Nutritional Services Continue practicing and providing other activities that is related to Nutritional

Health.

Vehicle This coming cycle 12 they should always be ready especially in the vehicle for the

distribution of goods.

Documents of Supplemental Feeding Program Provide own copy of Documents so if they have visitor’s they can provide

documents that needed.

I noticed that the other Day Care Workers have already improved. They must continue on what is being started on the good side so that the other Day
Care Workers will be motivated to improve their tasks.

RECOMMENDATION:

The undersigned told to ma’am Aquinillo (SF Focal) that this coming cycle 12 CY 2022, they should always be ready specifically in the mode of

transportation for the distribution of goods and hopefully all the requirements be prepared on time and be available.
