    SUBMISSION OF NUTRITIONAL STATUS REPORT

    TO : District In-Charge
    All Public Elementary and Secondary School Heads
    All others Concerned

    1. Submission of the Baseline Nutritional Status for school year 2021-2022 is required by the
    Regional Office. In this regard may we request assistance from the BHERTS of your
    respective barangays in taking the height and weight of learners since they are not
    reporting to schools yet. Raw data will be submitted to the School Nurses for computation
    and interpretation.

    2. The following data will be needed for the computation of the learners Nutritional Status
    Report:
    a) Full name of learner
    b) Grade level and section
    c) Learners’ birthday (month/day/year)

    3. For information and reference.
