AFFIDAVIT EXPLANATION
I, Pcms Benedicto S. Herrera, of legal age, married, and a resident of Brgy. 42- A
Rizal avenue Tacloban City after having been sworn to an oath in accordance to law,
hereby depose and say, to wit.
That I am member of Philippine National Police, presently assigned at Tacloban
City Police Office, Police Station 2, Downtown Police Community Precint Tacloban City
designated as patrol team leader PNCO.
That I received memorandum dated August 17, 2022 from ACD PCOL MICHAEL
P PALERMO, this pertains to the memorandum from chief RMDU 8 re absent during
Launching on Enhanced Monitoring of Police Overall Wellness (EMPOw} program
conducted on last July 20, 2022.
That I have no personal knowledge or information that said activity. In fact,
prior that day, July 19, 2022. I was 24H duty at Downtown PCP and I don’t received any
information from our admin personnel that I am one of the representative of that
activity ( EMPOw}.
That it was not my intension not to attend said activity, much less to defy or
undermined existing policies and other related instruction from Higher office.
That I humbly appeal that this explanation would merit for your kind and
humanitarian consideration.
That I am executing this affidavit for whatever legal purpose it may serve.
