Subject: CONTROL UNIT BREAKDOWN

Problem Description
On the 17th day of October 2022, our Speech Laboratory class was conducted. During
our class, the Control Unit fails, affecting all the units connected to it. While we investigate
what is the cause of the problem, we discovered that the device is already defective and the
cables were broken. Thus, the equipment must be thoroughly maintained every week to ensure
the safety of the students.

Action Taken
Based on the school and laboratory policy, the whole class must follow the laboratory
regulations while performing their duties. Because their students were involved in the accident,
the maintenance office must ensure that all equipment is in good condition. As a result, they
should take immediate action to replace the equipment.

Recommendation
1. The maintenance office should have a weekly inspection of all the equipments in the
school and make sure that they are in good condition.
2. Before the class used the laboratory, they must check it first to ensure the safety of
everyone.
3. After using the laboratory all used apparatus should be placed back where it was kept.
