Clearance Requirements

Office of College Secretary

Aside from college clearances, make sure that you have no other pending documents or
deficiencies in your respective office of college secretary.

● College of Arts and Sciences (CAS)

1. Download and accomplish the CAS-OCS Form No. 16 found in this link:
tinyurl.com/UPLBCAS-OCSForms
2. Send the accomplished form to academic/major adviser for signature (e-signature
is permitted). Your academic/major adviser should send the signed form to CAS-
OCS via email cas_ocs.uplb@up.edu.ph with subject: College Clearance.

● College of Engineering and Agro-Industrial Technology (CEAT)

1. Please accomplish the form CEAT College Clearance Online Application Form
For Graduates to request college clearance. Instructions regarding requirements
of different offices will be posted soon.

● College of Economic and Management (CEM)

1. Download and accomplish the College Clearance & Application for Graduation
Form (Answer the graduation form ONLY if you are graduating).
2. Student sends the college clearance form to his/her academic/major adviser,
Dept. Chair & CEM Librarian to obtain their signatures (e-signature is
permitted). See list below for concerned offices.

■ DAAE: daae.uplb@up.edu.ph
■ DAME: dame.uplb@up.edu.ph
■ DE: econ.uplb@up.edu.ph
■ CEM Librarian: cemlibrary.uplb@up.edu.ph

3. The student will send the accomplished College Clearance and Application for
Graduation Form to socem@up.edu.ph (Subject: College Clearance and
Application for Graduation Form). Wait for the acknowledgement receipt to be
sent by CEM-OCS.

1. Please send a copy of your accomplished college clearance to
che_ocs.uplb@up.edu.ph

Office of Anti-Sexual Harassment

● No pending sexual harassment case (If in the case that you do, please contact OASH to
settle your case.)

Office of Counseling and Guidance

Office of Scholarships and Grants

● If applies, Official Receipt/ Deposit Slip or Confirmation Slip as proof of payment for
SLB and Cash Loan

Student Disciplinary Tribunal

● If you have been meted out disciplinary measures, a certification from your respective
college secretaries must furnish the SDT, stating that you have already served, in full,
your penalty/ies.
