AFFIDAVIT TO SUPPLY MIDDLE NAME

I, LORFEL R. LAGARDE-PEÑARANDA, 46 years old, married and

a resident of Block 13 Lot 17 Villa Lolita Cogon San Jose Tacloban City,
after having been duly sworn to in accordance with law, hereby depose
and say:

1. That before marriage, my name maiden name was

LORFEL RAAGAS LAGARDE;

2. That I am the daughter of Spouses Felipe Lagarde and

Lolita Raagas;

3. That I was born on April 27, 1976 at the Tacloban City

Hospital Pericohon St. Tacloban City;

4. That my fact of birth was registered with the Local Civil

Registrar of Tacloban City, that however, there was no
entry for the middle name;

5. Since the last name of my mother is RAAGAS, I have been

using RAAGAS as my middle name ever since. Thus my full
name in my Certificate of Live Birth should be LORFEL
RAAGAS LAGARDE;

6. That I am executing this affidavit to attest to the truth of

the foregoing statement and to supply my middle name,
RAAGAS.
