PLEASE SAVE THIS FILE AS “GDI PL - Company Name - Product Name.doc” (Example:
“GDI PL - Company - Wood Panels.doc”)

PDS SUBMISSION REQUIREMENTS:

In order for this product to be assessed and approved for listing in Green Directory
Indonesia, please submit this PL Submission Form together with softcopies of ALL the
following:

1. Product Brochure/Specification Sheet

2. Certificates of all Green/Eco Labels obtained
3. High resolution logos of the Green/Eco Labels obtained (These will appear on the
Product Data Sheet - AI, JPEG & PDF formats are acceptable)
4. One (1) clear, high resolution image of the product to be featured on the PL.
(JPEG, PSD & TIFF formats are acceptable)

GDI may request for additional information/documents in order to complete the

assessment of the product if the materials provided above are insufficient or incomplete.
