The Applicant hereinbefore beg to state as under –
1. That the Applicant is the original complainant in the aforesaid case.
2. That the Applicant has lodged a criminal complaint against the
aforesaid accused at _______ Police station, for the offences
_______.
3. That pursuant to filing of said complaint, the said accused was
arrested by the concerned Police Officer; and the said accused was
produced before this Hon’ble Court.
4. That this Hon’ble Court was pleased to grant Bail to said accused
person, on strict terms and conditions, as mentioned in their Order of
Bail.
5. That the said accused is administering threats to the Applicant to
withdraw the case.
6. The Applicant therefore apprehends positive danger to life of
Applicant’s and Applicant’s family from the said accused.

7. That the said accused has breached the terms of the Bail so granted
by this Hon’ble Court and thus deserved to be withdrawn.
8. It is therefore, most humbly prayed that the Bail granted to the
accused be cancelled and he be taken into judicial custody.
