Please bring your own pen & ID upon claiming.

If you are sending a representative, please
provide letter of authorization with the ID of
the parent.
If you have several children and you wish to go
to the school to claim all the documents,
please be advised to go on the latest date that
appears on the tables below.
Please follow the scheduled date & time of
distribution for we are working on a skeletal
force.
Please observe the health & safety protocols
upon entering the school premises.

Original/printed copy of the Honor Certificates will only be issued upon request through this
email address - registrar.statefields@yahoo.com since these can be downloadable from the
SSi-Connect Parent Portal.
