Checklist of Additional Information

In addition to the completed client questionnaire, we must also receive all of the following information
so that we will have the data we need when we are preparing the legal forms which are required by the
U.S. government agencies we’ll interact with during the Green Card application process.

Each Green Card applicant must provide the following information:

▪ A copy of the identity page from your most recent passport


▪ A copy of your most recent U.S. visa stamp (only if applicable)
▪ A copy of your most recent Form I-94 Arrival/Departure Record (only if applicable)
▪ If you have family members (a spouse and/or children) who will also be receiving a Green Card,
we must receive a list of their names which includes their date and country of birth, and their
relationship to the primary applicant; plus a copy of the identity page from each of their passports.
▪ We must have your complete U.S. home address (if applicable) plus your address for your home
residence in your country of origin (not just the name of the foreign city or town where you live).
▪ We are required to explain where you have been living and working for at least the last 36 months,
so in addition to your home residence address we also need the business name, complete address
and phone number of every company you have worked for in your home country for the 36-month
period before you began your Green Card application process.
▪ Along with the name, address and phone number of any companies you worked for during the
last 3-years, we must also have the name of the supervisor or business owner at each company
you worked for, as well as your job title and a summary of your job duties at each company, and
the type of business each company is involved in.
▪ If you did not work anywhere during all or any portion of the last 36 months, then please provide
the dates that you were not working and the reason(s) for any periods of unemployment during
this 3-year period of time. Any reason(s) you may provide if you were unemployed during any
portion of this 3-year period of time will be acceptable. We must explain to the U.S. Department
of Labor what you have been doing for the last 3 years before starting your Green Card application
process with us, even if you were unemployed during all or any portion of that time.

All of the information listed above is required. We will not be able to prepare or file
your immigration forms until we have received all of the information listed above.
