IMPLEMENTATION OF COLA (CAVITE ONLINE LIS APPROVAL)

1. In pursuant to the Division Memorandum No. 120, s. 2020 dated March 16, 2020
titled “DepEd Cavite Province Preventive Measures Against COVID-19 Spread”, this
office thru the Planning and Research Section is implementing the utilization of COLA
(Cavite Online LIS Approval) to all public and private schools under this division.

2. COLA is an online system that allows the School Heads/LIS Coordinators to send
an online request for processing of all Division Level LIS requests such as follows:

 LRN Approval
 Change Request
 Request to Disapprove
 EBEIS/LIS Account Creation
 School Head LIS Account Assignment & Password Reset
 Correction of Basic Profile

3. All requesting Public and Private Schools are enjoined to send requests thru
COLA using the link bit.ly/SBS_COLA ensuring the completeness of all documentary
requirements stated in the forms.

4. Furthermore, in reference to the Division Memorandum No. 250 s. 2019 titled

“Division Guidelines on the Approval of Division Level LIS Requests” soft copy of the
revised Forms 1 to 6 can be downloaded through the links below:
