7.

Lightly palpate the external nose to determine any areas of tenderness, masses,
and displacements of bone and cartilage.
NURSE: I'm going to palpate your nose, please tell me if you feel ay pain.
RATIONALE: We start from the bridge of the nose right down the bony parts and in the
cartilaginous of the nose, the movable part of the nose we check if there's any deformity,
any pain, any tenderness of the nose.
FINDINGS: Nose is smooth and symmetric, with no tenderness upon palpation. My client
reports no pain.

8. Determine patency of both nasal cavities.

NURSE: Please close your mouth, and kindly block one nostril and then breathe through
the opposite nostril. And the opposite one please.

RATIONALE: The purpose of what I am doing is to determine patency of both nasal cavities, if
there is any nasal obstruction so that I can know if the air moves freely as the client breathes
through the nostril.
FINDINGS: There's no nasal obstruction found; my client was able to sniff through each
nostril while the other nostril is closed.

9. Inspect the nasal cavities using an otoscope with a short wide-tip attachment or penlight
and a nasal speculum.
Otoscope or; o Use your non-dominant hand to stabilize and gently tilt the client's
head back.

o Insert the short wide-tip of the otoscope into the client's nostril without touching
the sensitive nasal septum.

o Slowly direct the otoscope back and up.


Penlight and Nasal speculum

o Hold the speculum in your right hand to inspect the client's left nostril and your left
hand to inspect the client's right nostril.

o Tip the client's head back.

o Facing the client, insert the tip of the speculum about 1 cm (0.4 in.). Care must be
taken to avoid pressure on the sensitive nasal septum.
