By virtue of the authority vested in me by law, you are hereby notified that a complaint in
the above-entitled case has been filed against you and you are hereby directed to file your answer
and evidence in your defense 10 days from receipt hereof in accordance with Section 1 of R.A. 5180
as amended by P.D. Nos. 77 and 311 and to furnish the above named complainants with copies
thereof.

For purpose of you compliance, attached herein are copies of the complaint and the affidavit
of the following:

 S.S. of Yoshihiro F. Itabashi, Jr, S.S. of Carl S. Fronda and S.S. of Queenie Rae J. Ortiz

There are also other evidence submitted which shall be available for examination by you and your
counsel during office hours before the expiry date for you to file your answer with this office.

You are hereby WARNED that failure on your part to comply with this SUBPOENA shall be
considered as your WAIVER to present your defense in this Preliminary Investigation and the same
shall be considered submitted for Resolution.
