1 INTRODUCTION
This report is for record purpose intended to be kept as a lesson learnt process. It is
on Commuting Accident involving the Hired Staff Bus Reg. No. APP 182 HR driven by
Mr. Kunle Segun with 3 armed men along Sangotedo-Ajah Road, Lagos. The location
of the accident was at Majek Bus Stop Opposite NNPC Filling Station. The bus was
on official duty to pick-up staffs going to work from various designated locations within
Lekki-Ajah axis to V.I. Office and there was no passenger in the Bus as at the time of
Incidence.

The accident occurred on the 6th October, 2022 at 4.42 a.m.

The report is not intended to be used for punitive action.

After carrying out investigation, the Following findings were made;

 The driver embarked on his duty as early as 4.12am.

 The driver was crossed/ambushed by 3 armed men in a Toyota Camry car at

4.22am as he approached the scene of incidence - Majek Bus Stop Opposite
NNPC Filling Station.

 The driver was alert and swift in his response by Maneuvering the Bus to seek
for escape.

 The side window glass of the Bus was smashed with a golf stick by the armed
men while the driver was escaping.

 There was no passenger in the Bus as at the time of Incident.

3 FURTHER ACTION
The Leasing Company has since replaced the bus that had the accident with another bus
for use by the staff.
