JOINT-AFFIDAVIT

WE, JUANITA N. QUISEL, married, and MACARIO B. NEMARIA, JR., widow, both of legal
age, Filipinos, with residence and post-office address at Sta. Agueda, Pamplona, Negros
Oriental, under OATH, hereby depose and say that:

We personally know RAZIEL FELISARTA SAYCON, daughter of spouses Rafael Saycon Jr.
and Jovita Fibeing our close door neighbor and family friend in the community;

That FEB ADRIAN FELISARTA SAYCON was born on February 03, 2005 at Sta. Agueda,
Pamplona, Negros Oriental of the above-named mother;

That unfortunately, appearing in his Birth Certificate issued by Philippine Statistics

Authority, the name of his mother spelled and recorded as “RAZEIL F. SAYCON” instead of
“RAZIEL CALIBUGAR SAYCON” as his mother’s true and correct name;

That the names RAZIEL CALIBUGAR SAYCON and RAZEIL F. SAYCON refer only to one
and the same person whose true and correct name of his mother is “RAZIEL CALIBUGAR
SAYCON” the mother of Feb Adrian Felisarta Saycon;

We are executing this affidavit jointly in order to attest to the truth and veracity of the
above facts and to prove that names RAZIEL CALIBUGAR SAYCON and RAZEIL F. SAYCON refer
only to one and the same person, whose true and correct name is RAZIEL CALIBUGAR SAYCON
the mother of Feb Adrian Felisarta Saycon.
