Subject: Correction in the EPF/EPS account in r/o of Smt. Garima bearing EPF account
no. PN/25143/28.
Sir,

It is hereby submitted that Smt. Garima joined our school on 01/10/2005 and she worked
up to 30/11/2005 and left her services on her own accord.

That after the joining in our school and we deposited her EPF/EPS contribution in wrong
account number PN/25143/24 during the financial year 2005-06 respectively.

That the above noted mistake crept in unintentional, inadvertently and without any
wrongful gaining to himself.

That to save from further harassment and mental tension to the above said ex-member we
hereby request you to kindly make necessary correction in your record and transfer the
contribution from wrong account No. PN/25143/24 to her correct account no.
PN/25143/28 so that at the time of submission of withdrawal form for final settlement of
his EPF claim the concerned member should not be bereft from this benefit as provided
under the law.

We feel sincerely sorry for the inconvenience caused in this regard.
