Dear Parents/Guardians,

Greetings of Peace!
The PLV-NSTP-Civic Welfare Training Service will be having a symposium entitled, iKnowIt: One
Campus Symposium on Anti-Terrorism and Crime Prevention with the theme Safe Space
Environment for Youth. This event aims to inculcate public awareness for youth against
terrorism and crime related topic. It will be held on November 04, 2022, 8:00 AM to 12:00 NN
at PLV-Auditorium, Pamantasan ng Lungsod ng Valenzuela-Main Campus, Brgy. Maysan,
Valenzuela City.

In line with this, may we request for the personal presence of your child on the said venue and
date. Rest assured that we will strictly observe and implement the health and safety protocols
during this activity.

I am allowing my child to participate in the said event.

I am not allowing my child to participate in the said event.

This is not mandatory, but we will wholeheartedly appreciate your consent for our student to
attend the said duty.

WAIVER FOR ACTIVITY

Dear School Administrators, Faculty & Staff,

Pleased be informed that I have given my permission for my child to join the said activity:
“iKnowIt: One Campus Symposium on Anti-Terrorism and Crime Prevention”
November 04, 2022, 8:00 AM to 12:00 NN at PLV-Auditorium, Pamantasan ng Lungsod ng Valenzuela-Maysan Campus.

And I hold the school and its employees, free from liability for any loss or injury caused by any
untoward accident beyond their control, such as, but not limited to, fortuitous event, actions
of third persons, and actions of students in violation of schools, rules and regulations and safety
precautions.
