That I am the owner of business name registered to the Department of trade and Industry
as “ 4ever massage and spa “ with Business address purok 6 Brgy Mapalad sta. rosa Nueva Ecija
That I am closing the said business due to the Business Bankruptcy
That I have no financial obligation to any supplier or creditor during the course of the
operation of the said business .
That I am requesting the Department of Trade and Industry to cause of cancellation of my
business name registration:
That I am executing this affidavit for the purpose of testifying to the truth of the facts herein
contained and for the further purpose of requesting the authorities concern to issue one in lieu
thereof.
