accomplished CLIP (Closed Bank Incentive Program) signed by Anidel and Francis Achivida
together with duly validated PNB Payment Slip of the loan account of Sps. Helen and Fernando
Achivida with the closed All AsiaBank Corporation – Your Community Thrift Bank.

Please be informed that the collateral title, particularly TCT No. T-36054 registered in the name of
Helen S. Achivida(+) married to Fernando C. Achivida together with the corresponding
Cancellation of Mortgage is currently under process.

Further, kindly submit the following documents for our reference:

1. Signed Application Form for CLIP (Closed Bank Incentive Program) by the remaining heirs;
2. Notarized Extrajudicial Settlement of Estate with Special Power of Attorney;
3. Certified True Copy of Death Certificate and Marriage Certificate;
4. Certified True Copy of Birth Certificate of all children;
5. Photocopy of two (2) government-issued valid IDs (with photo and signature) of ALL the
heirs with two (2) signatures beside the photocopied IDs;
6. Photocopy of two (2) government-issued valid IDs (with photo and signature) of the
designated representative, if any; with two (2) signatures beside the photocopied IDs;
