TEACHING PLAN FOR HYPERTENSION DESCRIPTION OF THE LEARNER: Ms. Janet Lacambra, 48 years old, former government employee of Department Trade and Industry, was interviewed to assess her knowledge and awareness on how to maintain normal blood pressure. She is now currently taking in Amlodipine 5mg 1 tab OD and Losartan 50mg 1tab OD. She was just recently diagnosed of being hypertensive by her doctor. During assessment, her blood pressure ranges from 160/100 to 110/80 mmHg. She was also been diagnosed of Gastric cancer Stage 1 rule out liver metastasis and undergone surgery removing 80% of her stomach. She is eager to know what kind of diet or things she must do to prevent high blood pressure. She is a college graduate and able to speak English, Tagalog and Ilokano. She prefers a one-on-one discussion and used of English or Tagalog as medium of instruction. LEARNING NEED: How to manage Hypertension and its complication LEARNING DIAGNOSIS: Knowledge deficit: management of hypertension and its complication related to lack of information and resources. GOAL: The client will be able to perform skills that will help them prevent possible complications of hypertension.
