AFFIDAVIT OF LOSS OF MTN CARD/PACK

I, DANIEL AYODELE FEMI (Male) citizen of the Federal Republic of

Nigeria, residing at 32 Igbinoba street, Ikpoba Hill, Off Agbor Road,
Benin City, Edo State; hereby make an oath as follows:

1. That I am the declarant to this affidavit.

2. That I am the legitimate owner of the registered MTN sim card No.
08163896410
3. That the said MTN sim card was misplaced on the 18th October,2020
and the sim pack could not be found, hence this affidavit.
4. That this affidavit is for record purposes as mandated by law for the
retrieval of the said MTN sim card.

AND THAT I SWEAR to this affidavit conscientiously believing same to

be true and correct to the best of my knowledge and by virtue of the
provision of the Oaths Act 2004.
