BIOMETRIC INFORMATION COLLECTION AND USE

NOTICE AND CONSENT

GOBRANDS Inc. (the “Company”) collects certain biometric identifiers to facilitate an accurate
and efficient timekeeping system. The following biometric information will be collected: two sets
of fingerprints. This information will be used to log your entry and exit from your location of
work.

The biometric information GOBRANDS Inc. collects is shared only with those inside the Company
with a specific business purpose. It will not be shared with any third party, except for its data
processing contractor, unless (1) it first obtains appropriate written consent; (2) when required
by law; or (3) when disclosure is required by a subpoena or warrant. This information will be
destroyed within a reasonable period of time after your employment with GOBRANDS Inc. ends.

I acknowledge receipt of this notice and consent to the collection and use of my biometric data
under the terms described above.
