AFFIDAVIT OF UNDERTAKING
I, ISMAEL ASPE III, of legal age, Filipino, with residence at Zone 2 Sagrada, Barangay Carangcang,
Magarao, Camarines Sur. After having been duly sworn to in accordance with law, hereby depose and say,
to wit:

1. That my vehicle was involved in a Vehicular Accident that happened last March 29, 2022
entered into the PNP Blotter of Magarao Police Station at Magarao, Camarines Sur with
ENTRY No. 2022-03-0384 PAGE(s) 207;

2. That I hereby undertake to heed all notices and summons by law enforcements and bring my
motorcycle (a Bajaj Kawasaki with registration plate 1739EU and color blue sidecar) to serve
as material evidence and in cooperation with any pending investigation, whenever
necessary, and for the proper disposition of a case, if any should prosper;

3. That I am executing this sworn affidavit of undertaking accepting the abovementioned

responsibility of my own free will and volition with all ordinary & necessary consequences
that comes with it;

4. That I am executing this affidavit of undertaking in compliance with the policies, rules and
regulations set by PNP – Magarao; and

5. That I am willing to shoulder all the cost and incidental expenses to be incurred in relation
with this unless otherwise stated
