Dear Madame/Sir:

I would like to formally bring to your attention my concern over the termination charge for our Digitel Line
which we have decided to be disconnected for the following reasons:

1. Your failure to provide reliable line service – it was to our disappointment that we only had actual use
of your telephone for three months, yet, your records will reveal that we have reported to your
company on four occasions that our telephone had no dial tone. Please understand that the reliability
of communication line is crucial to our operations because of some concerns that are peculiar to
school operations (e.g. the need to call the parent of the student during emergency, the need for
communication line during typhoons, etc.). Such failure caused us to incur a lot of cell phone charges,
transaction delays, and complaints from the parents of our students.

2. Your failure to install the broadband connection – although I requested that the connection be
suspended until, June 2007, it was to my disappointment that your company failed to provide the
needed connection despite several promises given by your local business office. You must understand
that the internet connection is important so that we can effectively execute our lessons for the
computer subject, more particularly those that would need internet connection.

With this may I appeal that the disconnection charge be waived since my reason for such action is grounded on
reasons that points to the failure of your company to deliver the agreed upon service. It is in the interest of
customer service, ethical standards and fair business practices that consumers/subscribers be given the right
to choose the service provider that can satisfy their needs, which in this case, your company has failed to
provide.
