POLICIES AND PROCEDURES OF PHILIPPINE HEART CENTER

One of the existing policies of Philippine heart center is their quality policy, which
the Philippine Heart Center is dedicated to providing the highest level of comprehensive
Cardiovascular Care, Education, and Research. They pledge to meet all applicable
statutory and regulatory requirements. We are committed to constantly improving our
processes. In order to keep this promise, PHC will improve services on a continuous
basis in order to provide expert patient care with compassion. Create programs to
improve knowledge, skills, and risk-taking thinking. Promote patient-centered care,
respect, integrity, compassion, excellence, teamwork, and accountability in order to
advance stakeholders' interests. They may invest in its most valuable resource, like
human capital.

PHC's top management has delegated QMS-related responsibilities and

authorities. The ISO core team is mainly responsible and authorized for the creation,
implementation, and guidance of the QMS, as well as change management if
necessary. To guarantee that people are engaged, the members of organization are
ensures the conformity of quality management system. As well the ISO core team is
also responsible for assuring the effectiveness of the QMS. Second Customer focus is
must be promoted by the team of ISO core. In addition they are responsible for
maintaining the quality management system's integrity. Further, PHC’s Departments
shall retain documented information on the release of services which includes evidence
of conformity with acceptance criteria and traceability to the person’s authorizing the
release. Documented information includes, but is not limited to resolutions, memos,
turnover documents, and minutes of the meeting, discussion or presentation of projects,
contracts. The organization should keep documented information by using one or more
of the following methods: non-conformity report, action plan report, resolutions, memos,
meeting minutes, etc. Contracts or contract modifications
