AFFIDAVIT OF ONE AND THE SAME PERSON
I, MA. ISABEL C. LOPEZ, of legal age, Filipino, single, and a
resident of 157 España Boulevard, Sampaloc, Manila City,
after having been sworn in accordance with law, hereby deposes and states
the following:
1. I was born to spouses Mario Lopez and Sabel Lopez on 11 November
2003 in Harapin Ang Bukas, Mandaluyong City and was named in my
Certificate of Live Birth as Ma. Isabel C. Lopez;

2. Since grade school, I have consistently used in all my transactions,

personal or otherwise, the name MARIA ISABEL C. LOPEZ;

3. When I applied for a Philippine passport, I used the name Maria Isabel
C. Lopez;

4. In my Certificate of Live Birth, my name appears to be Ma. Isabel C.

Lopez;

5. I am executing this affidavit to attest to the veracity of the foregoing

statement to prove that Ma. Isabel C. Lopez and Maria Isabel C. Lopez
are one and the same person and for whatever legal purpose it may
serve.
