Sir/Madam:
I am seeking a medical assistance for PFC Darwin M. Lazaga, a personnel of the Philippine Army,
who was assigned to secure the retrieval of election paraphernalia of the BEI of Barangays Sta. Maria,
Tinuyop and Sipacong of Leon B. Postigo, Z.N. The said military personnel met an accident at about 10:30
in the evening of May 13, 2013. He is now confined at Sindangan General Hospital and will be brought to
Zamboanga del Norte Hospital, Dipolog City this afternoon.
