TO WHOM IT MAY CONCERN:

This is to permit my son/ daughter __________________________ to

participate/join the Crew Leaders Training Course (CLTC) to be held on November 4-
6, 2022 at Balaoang National High School, Paniqui, Tarlac.

I have considered the benefit that shall be derived from the participation of my
son/daughter in the Crew Leaders Training Course (CLTC) with the understanding that
the necessary safety measures/precaution shall be taken.

However, I shall not hold the BSP Tarlac Council Training Staff and school
personnel responsible for any untoward incidents that may happen beyond their control.
